Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities. Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 580 participants. We are currently seeking a Home Care Coordinator to join our Home Care team in Mattapan.

Role:

The Home Care Coordinator functions as an assistant to the Nurse Manager. The Home Care Department Coordinator assists with home care coordination, referrals and scheduling, communicates effectively with the Nurse Manger, Home Care Nurses and Home Care Aides and builds trust and fosters smooth relationships with Clients and their families as well as Home Care Staff.

Responsibilities:

  • Receives referrals for clients in need of home health services.
  • Builds trust and fosters smooth relationships with Clients and their families.
  • Communicates with the Home Care Nurse and the Clinical Nurse Manager as needed.
  • Interacts with Clients and family members in a manner that adheres to the highest standards of care and professionalism.
  • Adjusts home aide schedules for sick calls, vacations and other absences.
  • Understands individual participant home care needs and matches needs with home care aide strengths.
  • Distributes and files home care assessments.
  • Processes monthly home aide mileage reimbursement forms.
  • Tracks Home Health Aide attendance for timely payroll submission.
  • Files and copies document(s) as needed

Requirements:

  • High School Diploma/GED.
  • Certified Nurse Assistant (CNA) preferred.
  • Two (2) or more years of work experience in a home care, professional or business environment
  • Must possess the ability to think critically in order to solve problems
  • Excellent communication skills and customer service skills, organizational, critical thinking and problem solving skills.
  • Experience with Office 365 applications with a focus on Word, Excel and Outlook and Teams
  • Associates Degree in Healthcare related or Business field preferred
  • Understanding of Home Care services preferred

The salary range and/or hourly rate listed is a good faith determination of base compensation that may be offered to a successful applicant for this position at the time of this job advertisement.

When determining actual base salary and/or rate, several factors may be considered as applicable (e.g. location, years of relevant experience, education, training, and other factors as permissible by law). As such, most offers will not be at the top of the salary range.

Monday - Friday, 8:30 AM to 5:00 PM

40 hours weekly
