Job Summary

We are seeking a Home Care Coordinator to manage scheduling, intake, and care coordination for a senior living home care program in Connecticut. The coordinator will serve as the primary point of contact for patients, families, referral sources, and field caregiving staff.

Key Responsibilities

  • Manage intake and onboarding of new home care patients and clients
  • Schedule and coordinate home care visits for nursing and aide staff
  • Serve as primary liaison between patients, families, referral sources, and care team
  • Monitor care delivery compliance and follow up on missed or incomplete visits
  • Maintain accurate records in home care management software
  • Support billing, authorizations, and insurance verification processes as needed

Requirements

  • 2+ years of experience in home care coordination, scheduling, or patient services
