Licensed Practical Nurse (LPN) – Home Care

Position Summary:
We are currently seeking skilled and caring Licensed Practical Nurses (LPNs) to join our Home Care team on Oahu. This role is ideal for nurses looking for a flexible schedule with the opportunity to work one-on-one with clients in their homes. Part-Time and Per Diem positions are available.
Responsibilities:
- Provide hands-on nursing care in accordance with individualized care plans
- Conduct home visits to monitor client health and progress
- Administer prescribed medications and treatments
- Document observations and communicate any changes in client condition
- Collaborate with RNs, physicians, and other care team members
- Offer patient and caregiver education on basic health maintenance
- Ensure adherence to all safety and clinical guidelines
Qualifications:
- Active and unencumbered LPN license in the State of Hawaii
- Minimum of one (1) year of nursing experience (home care experience preferred)
- Current CPR (BLS) certification
- Strong clinical and organizational skills
- Compassionate, professional, and reliable
- Valid driver’s license, reliable transportation, and proof of auto insurance
