Licensed Practical Nurse (LPN) – Home Care

Location: Oahu, Hawaii

Compensation: Starting at $26.00/hr

Position Summary:
We are currently seeking skilled and caring Licensed Practical Nurses (LPNs) to join our Home Care team on Oahu. This role is ideal for nurses looking for a flexible schedule with the opportunity to work one-on-one with clients in their homes. Part-Time and Per Diem positions are available.

Responsibilities:

Provide hands-on nursing care in accordance with individualized care plans
Conduct home visits to monitor client health and progress
Administer prescribed medications and treatments
Document observations and communicate any changes in client condition
Collaborate with RNs, physicians, and other care team members
Offer patient and caregiver education on basic health maintenance
Ensure adherence to all safety and clinical guidelines

Qualifications:

Active and unencumbered LPN license in the State of Hawaii
Minimum of one (1) year of nursing experience (home care experience preferred)
Current CPR (BLS) certification
Strong clinical and organizational skills
Compassionate, professional, and reliable
Valid driver’s license, reliable transportation, and proof of auto insurance
