We are seeking a results-driven Home Care Marketer to represent a growing, patient-focused home care organization in the community and drive patient referrals and enrollments. This is a highly measurable, performance-based role focused on building relationships, generating referrals, and increasing starts of care (SOCs).
Success in this role is directly tied to your ability to develop and maintain strong referral relationships and consistently convert opportunities into active cases.

Partnering closely with families, healthcare providers, and community organizations, the team is committed to improving patient outcomes, reducing unnecessary hospitalizations, and delivering a seamless experience from referral through ongoing care.
Conduct field-based outreach to healthcare providers, community organizations, and referral sources
Build and maintain strong relationships with Medicaid and Private Pay referral partners
Generate consistent patient referrals and starts of care (SOCs)
Promote services and increase brand awareness in the community
Represent the organization at events, offices, and outreach initiatives
Identify and develop new referral channels and partnerships
Collaborate closely with intake, coordination, and HR teams to ensure smooth patient onboarding
Track activity and performance against monthly outreach and enrollment goals
Minimum 1 year of recent home care business development experience (recent experience is critical due to market changes)
Ideally currently working in home care or a similar healthcare outreach role
Proven ability to generate Medicaid and/or Private Pay home care referrals (excluding CHHA)
Strong relationship-building and communication skills
Ability to work independently in a field-based, results-driven environment
Familiarity with the NYC Metro home care landscape and referral networks
Comfort using social media for outreach and brand awareness (preferred, not required)
Valid driver's license and willingness to travel locally
Base Salary: $60,000 $100,000+ (based on experience and performance)
Compensation is adjusted based on SOC trends and overall performance
