Join Cornerstone VNA, an award-winning, mission-driven nonprofit dedicated to providing trusted, compassionate, expert care in the comfort of patients' homes. We're looking for a skilled and caring Registered Nurse (RN) to join our dedicated home care team serving the Hampton, NH community.

What You'll Do:
Manage a diverse home care caseload with autonomy and flexibility
Perform comprehensive nursing assessments and develop patient-centered plans of care
Collaborate with physicians and interdisciplinary team members to coordinate high-quality care
Monitor patient progress, adjust care plans, and educate patients and families on health management
Complete OASIS assessments with accuracy
Document care accurately and timely during patient visits
Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) as needed
Participate in case conferences and discharge planning
Follow the Nurse Practice Act, agency policies, and home health regulations
What You Bring:
Current NH or Maine Registered Nurse (RN) license or other compact state license (or eligibility)
Minimum 1 year of clinical nursing experience preferred (home care experience a plus)
Excellent clinical assessment, communication, and organizational skills
Ability to work independently while being part of a collaborative care team
Comfort working in a variety of home environments
Reliable transportation and valid driver's license (Mileage reimbursed!)
