Home Care Physical Therapist

Physical Therapist needed to provide one-on-one therapy care for patients in their home setting! Primary Job Responsibilities: Assess a person s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence. Collaborate with interdisciplinary team and colleagues to ensure a person s needs are met. Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests. Utilize a variety of therapeutic modalities to maximize care provided to clients Requirements: Vermont PT licensure Home Health experience and OASIS experience a must as this candidate will case manage McKesson/Netsmart EMR experience strongly preferred We are looking for a candidate with no requested time off.

Shift: Tuesday- Saturday 8-5
