Home Care Physical Therapist

Physical Therapist needed to provide one-on-one therapy care for patients in their home setting!

Primary Job Responsibilities:

• Assess a person s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
• Collaborate with interdisciplinary team and colleagues to ensure a person s needs are met.
• Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.
• Utilize a variety of therapeutic modalities to maximize care provided to clients

Requirements:

• Vermont PT licensure
• Home Health experience and OASIS experience a must as this candidate will case manage
• McKesson/Netsmart EMR experience strongly preferred
