Home Care Quality Intern

Essential Duties and Responsibilities
Quality and Performance Improvement
- Assist with quality improvement projects related to HHCAHPS, rehospitalization rates, and
overall quality performance metrics
- Support initiatives aligned with CMS Home Health Quality Measures and agency quality
goals
- Collect, organize, and analyze patient experience and clinical performance data
- Identify trends and opportunities to improve patient outcomes and satisfaction
- Assist in developing reports, dashboards, and educational materials
- Collaborate with interdisciplinary teams to support performance improvement efforts
Intake & Marketing
- Utilize HealthPivots and other data tools to identify and target potential referral sources
- Analyze market data to support growth and outreach strategies
- Assist in developing and strengthening relationships with referral sources (e.g., hospitals, skilled nursing facilities; Independent Living /Assisted Living communities; physicians, and community partners)
- Support Intake operations including referral tracking, data entry, and documentation
- Assist with outreach initiatives, marketing materials, and communication efforts
- Contribute to projects aimed at improving referral conversion and intake efficiency
- Other duties as assigned.
Minimum Qualifications:
Education:Current enrollment in a Nursing, Health Administration, Public Health, or related healthcare degree program at an accredited university
Experience: Prior internship, work, volunteer, or healthcare experience preferred,
but not required. Microsoft Office experience preferred.
