Home Care Registered Nurse Case Manager - RN

We are seeking a dedicatedFull-Time RN Case Managerto join our team in Indiana. In this role, you’ll trade the hectic hospital hallways for the comfort of your patients' homes, providing high-quality care where it matters most.

The Role & Schedule

As a Case Manager, you are the heartbeat of our patient care. You will oversee a dedicated caseload, ensuring comprehensive care coordination and clinical excellence.

• Status:Full-Time (40 hours per week).

• Territory:Patients are primarily located within the Indianapolis area. While most travel is within a 45-minute radius, flexibility for up to a 1 hour drive may be required occasionally.

Compensation & Benefits

• Annual Salary:$75,000 – $80,000.

• Mileage Reimbursement:$0.50 per mile.

What You’ll Do

• Perform initial and ongoing clinical assessments.

• Develop and implement personalized plans of care in collaboration with physicians.

• Educate patients and families on disease management and self-care techniques.

• Coordinate with a multidisciplinary team to ensure seamless transitions and holistic support.

• On-Call:Rotation is approximately every4–6 weeks(1 full week).

Qualifications

• Current, unencumberedRegistered Nurse (RN) licensein the state of Indiana.

• Valid Driver’s License, reliable transportation, and active auto insurance.

• Strong organizational skills and the ability to work independently.
