We are seeking a dedicatedFull-Time RN Case Managerto join our team in Indiana. In this role, you’ll trade the hectic hospital hallways for the comfort of your patients' homes, providing high-quality care where it matters most.
The Role & Schedule
As a Case Manager, you are the heartbeat of our patient care. You will oversee a dedicated caseload, ensuring comprehensive care coordination and clinical excellence.
Status:Full-Time (40 hours per week).
Territory:Patients are primarily located within the Indianapolis area. While most travel is within a 45-minute radius, flexibility for up to a 1 hour drive may be required occasionally.
Compensation & Benefits
Annual Salary:$75,000 – $80,000.
Mileage Reimbursement:$0.50 per mile.

What You’ll Do
Perform initial and ongoing clinical assessments.
Develop and implement personalized plans of care in collaboration with physicians.
Educate patients and families on disease management and self-care techniques.
Coordinate with a multidisciplinary team to ensure seamless transitions and holistic support.
On-Call:Rotation is approximately every4–6 weeks(1 full week).
Qualifications
Current, unencumberedRegistered Nurse (RN) licensein the state of Indiana.
Valid Driver’s License, reliable transportation, and active auto insurance.
Strong organizational skills and the ability to work independently.
