Job Title: Home Care RN
Location: Little Rock, AR 72211

Coverage Area: Pulaski, Saline, Jefferson, Faulkner counties
Work Schedule: Monday–Friday, 8:00 AM–5:00 PM; occasional weekends and on-call as needed
Pay: $54.00-$56.00 per hour
Scheduled pay: Weekly
Benefits: Health, Dental, Vision, Life Insurance, and Short term disability after 90 days

• Home Care RN Responsibilities

  • Pulaski, Saline, Jefferson, Faulkner counties
  • Perform comprehensive assessments of high-risk patients/members
  • Collaborate with primary care providers to implement individualized care plans
  • Identify, address, and continuously reassess appropriate and cost-effective levels of care
  • Coordinate non-medical support (ex: housing, transportation) to improve treatment-plan compliance
  • Coordinate care transitions with physicians, social workers, discharge planners, and other partners
  • Engage specialty resources as needed to support best outcomes
  • Maintain detailed documentation of clinical, functional, and outcome measures
  • Promote health education, illness prevention, and patient advocacy while protecting privacy/confidentiality

  Home Care RN Required Skills & Qualifications

  • Active Arkansas RN license and/or Compact RN required
  • Willing to travel within coverage area: Pulaski, Saline, Jefferson, Faulkner counties
  • 1+ year Home Health experience
  • OASIS, and Homecare Homebase (HCHB) experience required
