The Home Care RN Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Home Care RN Case Manager conducts day-to-day nursing activities to include assessment and education. The Home Care RN Case Manager for Saint Elizabeth Home Care is a part of a cohesive and collaborative interdisciplinary team with ample administrative support. Our Home Care RN Case Managers are equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.

Qualifications for a Home Care RN Case Manager:

  • Current unencumbered RN license in Rhode Island.
  • Minimum of one year nursing experience, home care experience preferred.
  • CPR certification.
  • Reliable transportation, current clean driver's license, automobile insurance.
