Job Summary

The Clinical Home Care Nursing Supervisor (Supervisor) is a pivotal leader responsible for the direct supervision of nurses and other vital members of the home health and home-based palliative care team, serving both adult and pediatric populations. This role demands exceptional clinical expertise, visionary leadership, and strategic oversight to ensure the delivery of outstanding patient care. The supervisor orchestrates staff scheduling, upholds productivity standards, and possesses the authority to approve essential overtime, directly influencing operational efficiency, and patient outcomes.

As a cornerstone of team development, the supervisor leads the hiring, orientation, and comprehensive training of all team members, fostering a culture of excellence and continuous learning. Acting as a dynamic resource and mentor, the supervisor empowers staff, guiding them through complex clinical scenarios, and professional growth opportunities. Under the direction of the Home Care Services Clinical Operations Manager, the supervisor engages in high-level interprofessional problem-solving, spearheads performance improvement initiatives, and ensures unwavering compliance with regulatory standards. This includes conducting staff competency assessments and delivering thorough annual performance evaluations.

This supervisor must be a registered nurse with a robust foundation of clinical knowledge, particularly in home-based health services including acute and chronic pediatric care, palliative management of chronic conditions, and compassionate end-of-life care. This role requires exceptional judgment, decisive decision-making abilities, and advanced organizational skills to direct nursing operations effectively. Strong interpersonal skills are essential to facilitate seamless communication and collaboration across disciplines.

Accountability and a commitment to excellence define this position. The supervisor takes ownership of their own nursing practice, seeks continuous learning opportunities, and actively pursues professional development. Fundamental responsibilities include strategic planning, meticulous coordination, and rigorous evaluation of nursing and clinical activities to deliver comprehensive, patient-centered, interprofessional care. Moreover, the supervisor identifies and implements evidence-based nursing interventions, evaluates their impact on diverse patient populations, and acts as an indispensable resource for the home health and palliative care teams.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position

• American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) or UC Davis Health CPR
• Required to hold a valid California driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program
• Graduation from an accredited school of nursing with a Bachelor of Science in Nursing (BSN)
• At least three (3) years of recent Home Health RN experience
• At least one (1) year of recent Home Health and Home-Based Palliative Care RN experience in a leadership role
• Ability to read, write, speak, and understand the English language.

Department Overview

The UC Davis Health Home Care Services Department (HCS) is a full-service agency offering the best in home health care. Our Home Health Program is nationally ranked among the highest in patient satisfaction. Our mission is to provide exemplary home care that responds to the diverse physical, psychosocial, and spiritual needs of patients during the healing process.

Nurses provide care to patients ranging in age from pediatrics through geriatrics and patients have a large variety of medical needs. Patient conditions may include both acute and chronic conditions spanning across many care specialty areas. The focus of the nurses' in-home care is providing patient-centered care by supporting patient and families/caregiver goals.

Department Specific Job Scope

The primary responsibilities for this position consist of the oversight of total, comprehensive, continuous, and patient-centered care which includes assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive home health/home-based palliative care plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to university policies and procedures as well as federal, state, and local regulations.

Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with American Nurses' Association Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.

The Home Care Services (HCS) department is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Home-Based Palliative Care. This position is part of the operations leadership team and supervises the in-home services programs: home health (including pediatrics) and home-based palliative care.

Work Environment
