We are a mission-driven senior care organization focused on helping elderly participants remain safely in their homes through proactive, interdisciplinary, home-based healthcare support. Our nursing team plays a critical role in reducing unnecessary hospitalizations, supporting urgent participant needs, and delivering compassionate care directly in the community.

We are actively expanding our nursing team and seeking a Weekend Field RN who enjoys autonomy, geriatric care, independent decision-making, and meaningful patient relationships outside of a traditional hospital setting.

Job Details

We are seeking a Weekend Field RN to provide hands-on nursing support for elderly participants in home-based and community settings. This role is ideal for an RN who is comfortable working independently, making sound clinical decisions in the field, and supporting urgent participant needs with compassion and confidence.

Schedule:
Saturday: 7 AM – 11 PM
Sunday: 7 AM – 11 PM
Two 16-hour shifts

Responsibilities:
  • Perform in-person participant assessments to determine appropriate levels of care
  • Respond to urgent, emergent, and non-urgent participant needs
  • Complete regular home visits and field-based nursing visits
  • Provide wound care, IV hydration, lab draws, assessments, and other nursing tasks
  • Respond promptly to calls from providers, triage, and support staff
  • Educate participants, family members, caregivers, and facility staff
  • Coordinate with providers, home care, hospitals, pharmacies, DME suppliers, ALFs/SNFs, and other care partners
  • Initiate emergency treatment when needed
  • Document calls, visits, orders, incidents, falls, medication errors, and quality-related items
  • Participate in quality improvement initiatives and team communication
  • Adjust to changing workloads and priorities based on participant needs

Qualifications:
  • Graduate of an accredited school of nursing
  • Current RN license in the State of Michigan
  • 1–2+ years of home care, geriatric, long-term care, hospice, or field nursing experience preferred
  • Experience working with frail elderly or long-term care populations
  • Strong clinical assessment and critical thinking skills
  • Comfortable working independently in the field
  • Strong written and verbal communication skills
  • Proficiency with Microsoft Word, Outlook, and EMR documentation
  • Current CPR certification or willingness to obtain
  • Reliable transportation, valid Michigan driver’s license, and car insurance
  • Comfortable driving extensively for home visits
