The individual in this role is responsible for coordinating the scheduling and tracking of home care and home care assistant assignments. This position requires providing outstanding customer service to all stakeholders, including participants, families, vendors, and staff, while maintaining current and accurate records of all scheduled appointments. Collaboration with colleagues and stakeholders is essential to promote organizational values, team cohesion, and culture.
Job Responsibilities
Effectively coordinate the scheduling of home care personal care assignments, including adjusting schedules based on changes and communicating with all stakeholders.
Ensure required authorizations in the electronic medical record (EMR) for approved personal care.
Ensure all care logs and timesheets are received from vendors and in-home staff visits.
Implement procedures to ensure a successful experience for providers, staff, and participants, such as reminder calls and sending materials ahead of time.
Maintain current and accurate records of all scheduled home care personal care assignments, including both the scheduled times and outcomes.
Be willing and able to work a varied schedule that may include evenings, nights, weekends, and overtime.
Complete all required documentation in a timely and accurate manner.
Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
Follow company policies and procedures and participate in any required quality improvement activities, staff training, and meetings.
Communicate regularly with supervisors and staff regarding workload and priorities.
Ensure timely completion of all mandated trainings and education.
Ensure timely completion of all mandated occupational health screenings as needed.
Exercise flexibility in performing assignments as business needs evolve.
Perform other duties as assigned.
Required Skills & Experience
Minimum of one year of experience as a scheduler in a home health or home care agency.
Ability to process accurate information in a timely manner.
Ability to follow directions from supervisors and communicate needs effectively.
Excellent organizational and communication skills.
Ability to work independently with minimal supervision.
Demonstrated ability to prioritize and manage staff or demands in a fast-paced environment.
Experience and competency working with people from diverse backgrounds and cultures.
Commitment to unlocking the full potential of vulnerable seniors.
Preferred Skills & Experience
Minimum of one year of experience working with an electronic medical records and scheduling system.
Schedule Notes
8-week assignment, Monday to Friday, 40 hours a week.
