*Position Summary*
The Speech Pathologist provides one‑to‑one, patient‑centered care using advanced technology and clinical expertise in the home setting. Care is delivered through a multidisciplinary approach, collaborating with physicians, nursing staff, social workers and home health aides.
Candidates must be highly motivated, detail‑oriented, organized, technologically proficient and comfortable working autonomously.
*Responsibilities*
* Provide individualized speech therapy services in patients' homes

* Collaborate with interdisciplinary team members to support comprehensive care plans
* Utilize advanced and easy‑to‑use clinical technology
* Maintain accurate clinical documentation

*Minimum Qualifications*
* Meets educational and experience requirements for *Certificate of Clinical Competence in Speech Pathology (CCC‑SLP)* from ASHA

* Meets educational requirements and is currently completing the supervised experience required for certification, registration or licensure in the State of Illinois (if applicable)
