University Home Care Inc is seeking a dedicated Home Care Speech Therapist to provide high-quality speech therapy services to clients in their homes in the Southeast MI Region. Must be willing to travel. Responsibilities
- Evaluate, assess, and diagnose speech, language, cognitive-communication, and swallowing disorders.
- Develop and implement individualized treatment plans tailored to each patient’s needs.
- Provide direct speech therapy services to clients in their homes with a focus on functional outcomes.
- Document patient progress and maintain accurate clinical records in compliance with company policies and regulatory requirements.
- Collaborate with healthcare professionals, families, and caregivers to support optimal patient care.
- Participate in ongoing professional development and training to stay current in the field.
Requirements
- Valid Speech-Language Pathologist (SLP) license or certification as required by the state of Michigan.
- Experience in home health or outpatient speech therapy preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation to travel to clients’ homes within Southeast MI Region.
- Strong organizational skills and attention to detail in documentation.
