The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.
- 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating Weekends and Holidays

Job Details:
Our Home Care Transition Nurses:
- Conduct patient assessments and develop individualized transition plans
- Coordinate referrals and facilitate smooth transitions to home care services
- Build and maintain strong relationships with physicians, discharge planners, and referral sources
- Serve as a key ambassador for Catholic Home Care within the community
- Identify opportunities for program growth and service expansion
- Deliver exceptional customer service and support to patients, families, and healthcare partners
- Collaborate with interdisciplinary teams to ensure positive patient outcomes
Requirements:
- Current Registered Nurse (RN) license in New York State
- Minimum of 2 years of home care experience, including patient assessments
- Strong relationship-building, communication, and customer service skills
- Experience in healthcare marketing, referral development, or business growth is a plus
- Passion for patient advocacy and care coordination
Posted Salary Range:
USD $95,000.00 - USD $120,000.00 /Yr.
