Make a lasting impact by supporting patients through a safe and successful transition home The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals at St. Joesph's hospital in Bethpage. 

 

The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.

• 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating Weekends and Holidays

Job Details:

Our Home Care Transition Nurses:

• Conduct patient assessments and develop individualized transition plans
• Coordinate referrals and facilitate smooth transitions to home care services
• Build and maintain strong relationships with physicians, discharge planners, and referral sources
• Serve as a key ambassador for Catholic Home Care within the community
• Identify opportunities for program growth and service expansion
• Deliver exceptional customer service and support to patients, families, and healthcare partners
• Collaborate with interdisciplinary teams to ensure positive patient outcomes

 

Requirements:

• Current Registered Nurse (RN) license in New York State
• Minimum of 2 years of home care experience, including patient assessments
• Strong relationship-building, communication, and customer service skills
• Experience in healthcare marketing, referral development, or business growth is a plus
• Passion for patient advocacy and care coordination

Posted Salary Range: USD $95,000.00 - USD $120,000.00 /Yr. :
