    Description

    We are seeking a motivated and caring Home Health Aide CNA (Certified Nursing Assistant) to join our team for one of our clients in Livonia / Northville.
    In this vital role, you will provide essential personal care and support to clients in their homes, ensuring their safety, comfort, and well-being.

    Responsibilities

    • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
    • Monitor patient health by observing vital signs and reporting any changes to the healthcare team
    • Provide medication reminders and assist with medication administration following proper protocols
    • Support clients with mobility aids such as Hoyer lifts and assist with transfers safely
    • Document patient care activities accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance
    • Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment
    • Observe and report any changes in mental status or behavior related to dementia or Alzheimer#39;s care

    Experience

    • Prior experience working in home health or senior care settings preferred
    • Experience assisting individuals with developmental disabilities or memory care needs beneficial
    • Knowledge of medical terminology, catheter care, hospice care, and medical documentation required
    • Familiarity with EMR (Electronic Medical Records) systems and HIPAA regulations essential
    • CPR certification required; additional certifications such as medication administration or dementia care are a plus
    • Long-term care experience in nursing homes or assisted living facilities is advantageous
    • Ability to work independently while providing compassionate patient-centered care
