We are seeking a motivated and caring Home Health Aide CNA (Certified Nursing Assistant) to join our team for one of our client's in Livonia. In this vital role, you will provide essential personal care and support to clients in their homes, ensuring their safety, comfort, and well-being.

Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patient health by observing vital signs and reporting any changes to the healthcare team
  • Provide medication reminders and assist with medication administration following proper protocols
  • Support clients with mobility aids such as Hoyer lifts and assist with transfers safely
  • Document patient care activities accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance
  • Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment
  • Observe and report any changes in mental status or behavior related to dementia or Alzheimer's care

Experience

  • Prior experience working in home health or senior care settings preferred
  • Experience assisting individuals with developmental disabilities or memory care needs beneficial
  • Knowledge of medical terminology, catheter care, hospice care, and medical documentation required
  • Familiarity with EMR (Electronic Medical Records) systems and HIPAA regulations essential
  • CPR certification required; additional certifications such as medication administration or dementia care are a plus
  • Long-term care experience in nursing homes or assisted living facilities is advantageous
  • Ability to work independently while providing compassionate patient-centered care
