Tides Hospice is seeking a compassionate, dependable and caring Home Health Aide (HHA) to join our team. The Hospice HHA provides essential personal care and supportive services to terminally ill hospice patients, helping promote comfort, dignity, and quality of life throughout the hospice journey. Services are provided in Assisted Living facilities and home-based settings. The HHA works under the direction and ongoing supervision of a Registered Nurse (RN) and as an important member of the Hospice interdisciplinary team. Position Responsibilities Under the direction and ongoing supervision of the RN, the Hospice Home Health Aide will: Provide personal care, including bathing, oral care, skin care, and hair care. Assist patients with toileting, bedpans, and commodes. Assist with bed transfers and ambulation as outlined in the patient's care plan. Perform range-of-motion exercises as instructed by qualified professional personnel. Prepare meals, including meals for patients requiring special diets. Perform essential incidental household tasks that support the patient's comfort and well-being. Assist with shopping when the patient has no other available resource. Take temperature, pulse, respirations, and blood pressure when assigned by the RN. Provide compassionate emotional support to patients and their families. Demonstrate active listening and offer encouragement when appropriate. Maintain patience, kindness, and professionalism in all interactions. Immediately report changes in the patient's condition or family situation to the Hospice office and RN. Follow the patient's individualized care plan and all Tides Hospice policies and procedures. Practice Standard Precautions and follow OSHA and infection-control requirements. Maintain required competencies and skills. Participate in required RN supervision and evaluations, including ongoing supervision every 14 days for each case assignment. Participate in annual performance evaluations. Complete at least 12 hours of in-service education each year . Maintain current certification and credentials as required by the state. Maintain current knowledge of infectious disease protocols and infection-control practices. Work collaboratively with the Hospice interdisciplinary team to provide quality end-of-life care.
