Position Overview

As a Home Health Aideyou will provide unskilled services tailored to each client’s needs, following individualized care plans all in a comfortable home environment.

Key Responsibilities

• Deliver direct, one-on-one nursing care in the patient’s home
• Follow individualized plans of care
• Monitor patient condition and document progress
• Monitor for safety
• Communicate changes with clinical care team
• Maintain accurate and timely clinical documentation

Qualifications

• Proof of training OR we can also train you if you do not have training)
• Ability to pass a background check and drug screening
