Home Health Aide (HHA)

Champlain Valley Senior Community is seeking compassionate and dependable Home Health Aides (HHAs) to provide person-centered care to residents in our Assisted Living, Enhanced Assisted Living, and Memory Care community.

Responsibilities:

• Assist residents with personal care, including bathing, dressing, grooming, toileting, and mobility

• Provide reminders and assistance according to each resident’s individualized service plan

• Respond promptly to resident needs and call bells

• Observe residents and report changes in condition to the appropriate supervisor

• Assist with meals, hydration, transfers, repositioning, and other activities of daily living

• Complete required documentation accurately and before the end of each shift

• Maintain residents’ privacy, dignity, safety, and independence

• Follow infection-control procedures and all facility policies

• Work cooperatively with nurses, medication technicians, resident care assistants, and other team members

• Participate in required orientation, training, and staff meetings

Qualifications:

• Current New York State HHA certification required

• Compassionate, dependable, and able to work effectively with older adults

• Experience in assisted living, memory care, or senior care is preferred

• Ability to communicate clearly and follow written care instructions

• Ability to perform the physical duties of the position safely

• Must successfully complete all required background checks, health requirements, and employment documentation

Schedule:

Full-time, part-time, and per-diem opportunities may be available. Availability for evenings, weekends, and holidays may be required based on the position.
