Lakeview is looking to hire a Home Health Aide to join our Homecare/Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
Assists professional personnel to provide patient care according to department and hospital standards. Completes other supportive tasks necessary to the functioning of the department under the supervision of a Case Manager.
Essential Duties:
Communicate pertinent data clearly, concisely and in a timely manner to the Case Manager.
Maintain department order and equipment management as needed.
4.Performs other tasks as required or assigned.
6.Driving to client's residence.
Work Schedule:
0.6 FTE (scheduled 3 days per week). Working hours are 8-4:30 pm. No weekends or holidays.
Required Qualifications:
Education
High school diploma or equivalent
License and Certification:
Must have a valid driver's license and meet all criteria required under Lakeview's Automobile Liability Guidelines
Must have MN or WI CNA certificate upon start date
Must provide proof of MN and WI CNA certificates within 4 months of start, if both MN and WI CNA certificates are not present on start date
Current BLS - American Heart Association or BLS - American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
Experience / Skills
Must be 18 or older and have at least 1 year of driving experience
