Job Summary
We are seeking a Home Health Aide or Certified Nursing Assistant  to join our team! As a Home a Health Aide or Certified Nursing Assistant on our team, you will be providing personal care, assisting with light house chores, provide meal assistance and feeding, medication reminder, escorting clients to medical appointment and maximizing quality of life which is unique to every client or patient and family. The ideal candidate has excellent communication and interpersonal skills, a strong understanding of compassionate care, empathy, attentive, awareness and the ability to multitask and switch focus, as needed, to provide the best care for each patient. 

Responsibilities 

• Warmly greet each patient that walks into your office, listening to their concerns and asking questions to learn about their lifestyle and health issues
• Counsel patients suffering from a variety of health issues on changes they can make in their daily lives to improve their nutrition
• Provide meal plans and recipe suggestions that are in line with their ultimate health goals
• Regularly communicate with existing patients and check in on their progress
• Be a helpful, supportive presence for patients looking to make major changes in their life

Qualifications

• Must hold the required license in the state
• Excellent communication and interpersonal skills
• Strong understanding of patient’s needs as per supervision of the RN
• The ability to provide transportation assistance (hoyer lift, bed to chair assistance, showering)
• The ability to work independently and within a team environment
• Contience Care (including Foley catheter and colostomy care)
• Meal preps and feeding assistance
