Home Health Aide / Personal Care Aide (Certified) Per Diem & Live-In Cases Long Island, Nassau

Looking for meaningful, flexible work caring for clients close to home? Alvita Care is growing our team of dependable caregivers across Nassau County and nearby areas. We have per diem shifts and live-in cases available. Experience in a home or facility setting is a plus, but we value kindness and reliability above all.

New York State certification is a must. To be considered, you need a current NYS Home Health Aide (HHA) or Personal Care Aide (PCA) certificate. If you're not yet certified, we can't move forward with your application.

What you'll earn: $19.65-$22.00 per hour, depending on the case

What we're looking for:

• Current NYS HHA or PCA certificate
• Legal authorization to work in the U.S.
• A dependable, professional, and compassionate approach to care
• Respect for client privacy and discretion in all communication
• Up-to-date medicals and vaccinations (or willingness to get them)
• A valid driver's license and your own transportation are a plus
• A completed drug screen or Habituation Form
• Completion of Alvita Care onboarding before accepting cases

About Alvita Care
We're a mission-driven home healthcare and personal care agency with 4 branches and more than 500 caregivers. Our services include in-home personal care, Alzheimer's and dementia care, palliative and hospice care, and geriatric care management. We hold The Joint Commission Gold Seal of Approval, and all of our clients are private-pay (no Medicare or Medicaid).

Caregivers can work private-duty one-on-one cases (including live-in) or facility-based shifts, including per diem. Every applicant completes a phone screening so we can find the right match for both caregiver and client. Cases are offered only to caregivers who have completed onboarding.
