Job SummaryWe are looking for a
Home Health Aide to join our team! You will directly work with residents, following a one-on-one care plan at our assisted living home. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
Responsibilities - Assist residents with activities of daily living such as preparing meals, showers, moving in and out of beds,
- Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
- Maintain records of resident care, condition, progress, or problems to report and discuss observations with the supervisor
- Build relationships with residents by conversing or reading aloud to help keep them mentally healthy and alert
- Maintain a clean and neat environment of the home.
- Follow policies and procedures as set by the Department of Health and Human Services.
- Provide transportation for residents as needed.
- Give medications as prescribed by health care professionals and submit provide accurate documentation on a timely manner.
- Engage residents in exercises or other activities
- Be computer literate.
Qualifications- Undergraduate, or Associate Degree is a plus.
- High School Diploma or GED
- One-year prior professional experience
- Strong verbal and written communication
- Driver’s license required
- CPR and First Aid certification required, and TB testing during hiring.
