Home Health and Hospice Admission Nurse

Position Summary
The Home Health and Hospice Admission Nurse is responsible for evaluating patients for admission, completing comprehensive assessments, developing initial care plans, and coordinating services to ensure patients receive appropriate home health or hospice care. This role serves as a key liaison between patients, families, physicians, and the interdisciplinary care team.

Key Responsibilities:
  • Conduct initial patient assessments and admissions in home or facility settings.
  • Review medical records and physician orders to determine eligibility for services.
  • Complete required admission documentation accurately and timely.
  • Develop and implement individualized plans of care.
  • Educate patients and families regarding diagnoses, treatments, medications, and available services.
  • Coordinate care with physicians, case managers, social workers, therapists, and other healthcare professionals.
  • Ensure compliance with Medicare, Medicaid, accreditation, and agency requirements.
  • Evaluate patient needs and recommend appropriate services and resources.
  • Participate in on-call rotations as assigned.
  • Maintain patient confidentiality and uphold professional nursing standards.

Qualifications:
  • Current Registered Nurse (RN) license in good standing.
  • Graduate of an accredited nursing program.
  • Home health, hospice, case management, or admissions experience preferred.
  • Strong assessment, communication, and organizational skills.
  • Knowledge of Medicare, hospice, and home health regulations.
  • Valid driver's license and reliable transportation.
