Location: Northern California (multiple counties)
Overview:
We are seeking a motivated and experienced Healthcare Marketing Liaison to support the growth of our home health and hospice services. This role is responsible for building and maintaining strong relationships with referral sources, including hospitals, skilled nursing facilities, assisted living communities, and physicians.
Responsibilities:
• Develop and maintain referral relationships with healthcare providers and community partners
• Promote home health and hospice services to referral sources
• Conduct regular in-person visits to hospitals, SNFs, ALFs, and physician offices
• Identify new business opportunities and expand service areas
• Coordinate with clinical teams to ensure smooth patient transitions
• Track referrals and maintain accurate marketing activity logs
• Represent the agency professionally in the community
Qualifications:
• Experience in healthcare marketing, home health, or hospice preferred
• Strong communication and relationship-building skills
• Knowledge of Medicare and referral processes is a plus
• Ability to work independently and meet growth goals
• Reliable transportation required
