Full-Time Home Health Assistant Clinical Registered Nurse (RN) Manager Requirements and Schedule:

• Status: Approximately 40 hours per week, exempt salaried position
  • Exempt salaried position definition: You will receive a predetermined salary with expectations that your objectives and duties will be met, regardless of the exact number of hours worked in a given week. Exempt employees are those who are exempt from minimum wage and overtime pay requirements. This is because exempt employees are paid a salary rather than an hourly wage, and they work in what are considered executive or professional jobs. On the contrary, a non exempt employee is one that gets paid hourly where each hour is paid.
• Office Hours: Monday – Friday, approximately 0800 - 1600
• Work Environment: Hybrid/Flexible (Office & Field). Patient visits and field responsibilities take priority over office-based tasks.
• On-Call Responsibilities (Alternating)
   
  • Weekday On-Call (Alternating): Monday – Friday, 1600 - 0800
  • Weekend On-Call Non Resource (Alternating): Friday 1600 – Monday 0800
  • Weekend On-Call Resource (Alternating): Friday 1600 – Monday 0800
  • Holiday On-Call (Alternating)

• Additional Duties: Additional duties include community outreach, attending marketing lunch and learns and presentations, visiting referral source offices, visiting medical providers' offices to gather signed orders, etc.

Full-Time Home Health Assistant Clinical Registered Nurse (RN) Manager Compensation:

• Orientation Period: $30.00 / hour
• Post Orientation Period Annual Salary: $85,000
• Weekday, Weekend (Non Resource) & Holiday On-Call Compensation:
  • $200 per regular two-day weekend 
  • $100 per day for single-day weekend on-call (Friday/Saturday or Sunday/Monday)
• Weekday, Weekend & Holiday On Call Visit Pay: $60 per point
  • 1 Point: Regular Visit, Discharge with Visit, Evaluation Visit 
  • 1.33 Points: Recertification OASIS Visit, Resumption of Care OASIS Visit, Other Follow-Up OASIS Visit 
  • 1.67 Points: Start of Care OASIS Visit
• Mileage Reimbursement (Non-Taxed)
  • Business Days (Monday-Friday)
    • Field Visits Only: $0.50 per mile after 30 miles per day round trip from your home back to home 
    • Office Work Only (Northville Location): No mileage reimbursement 
    • Mixed Day (Office & Field Visits): $0.50 per mile after your "Normal Commute" miles per day round trip from your home back to home
  • On-Call Weekdays, Weekends & Holidays: $0.50 per mile round trip from your home back to home (no 30-mile deduction)

Home Health Assistant Clinical Registered Nurse (RN) Manager Job Description:

• Provide care to patients in the community to help them get back to their life of independence.
• Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
• Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, or upon changes in the patient’s condition.
• Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition.
• Exhibits critical thinking and applies them for continuous improvement of services and the agency
• Assistance to the Clinical Manager(s) and Director of Clinical Services
• Coverage for office staff

Home Health Assistant Clinical Registered Nurse (RN) Manager Requirements:

• Active Michigan Registered Nurse (RN) license
• Reliable transportation
• Valid driver's license and auto insurance in your name as a driver
• Home health care experience preferred

Home Health Assistant Clinical Registered Nurse (RN) Manager Degree of Travel:

• Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
