    COMMUNITY LIAISON – HOME HEALTH

    Job Location: Dickson, TN

    Office: Home Care Solutions

    Status: Full - Time

    POSITION SUMMARY: The Community Liaison assists the business development and marketing leaders in communicating with hospitals, skilled nursing homes, board and care homes, outpatient discharge planning, and physician offices. Provides and acts as a community resource and public relations representative in facilitating safe, quality healthcare in our communities. Assists business development and marketing leaders to generate appropriate patient referrals/admissions by establishing and maintaining professional relationships with all referral sources.

    ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

    • Generates business by creating new and maintaining existing relationships with physicians and other medical professionals, community and senior organizations, hospitals, appropriate special interest groups, discharge planners, nursing home professionals, assisted living facilities, and other potential referral sources.
    • Contacts, prepares, and conducts sales and educational presentations for physicians, businesses, and local community groups regarding home care services and benefits.
    • Assists the Director of Business Development in establishing organization volume projections in the annual budget and in establishing allocations for the marketing department. Monitors allocation of resources according to budgetary limitations.
    • Conducts market assessments and develops a comprehensive marketing plan designed to meet budgetary volume projections.
    • Exceeds client and internal expectations by providing daily communication and creative solutions to problems and bringing a positive attitude.
    • Establishes and maintains positive working relationships with current and potential referral and payer sources.
    • Documents all physician office visits, referral source visits, follow up, phone calls and gifts. A minimum of 50 documented quality interactions per week.
    • Ensures all communications, messaging and branding is aligned with the agency culture.
    • Builds and monitors community, customer, payer, and patient perceptions of the agency as a high quality service provider.
    • Strategic planning including identifying opportunities for additional or improved services to address customer needs.
    • Maintains comprehensive working knowledge of home health and hospice markets including government agencies, major payer groups, key referral sources, and competitor’s market positioning.
    • Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate organization personnel.
    • Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by the agency.
    • Monitors and reports cost effectiveness of marketing efforts.
    • Other appropriate tasks and special projects as assigned.

    QUALIFICATIONS & REQUIREMENTS FOR THE POSITION:

    • Bachelor's degree in Marketing, Business Administration, or related field.
    • At least three (3) years of experience in health care marketing preferably in home health or hospice operations.
    • Demonstrated ability to work independently to increase business targets, obtain contacts, and maintain relationships in the communities.
    • Ability to market and deal tactfully with customers and the community.
    • Able to maintain an organized approach to territory management.
    • Outstanding verbal and written communication skills.
    • Excellent presentation, negotiation, and relationship-building skills required.
    • Must have strong computer skills to utilize Microsoft Outlook and other agency systems.
    • Self-motivated; able to work independently with minimal oversight.

    • Job may be performed at a desk and require heavy use of office equipment, including computer, tablet, and phone.
    • Must have the reasonable ability to stand, walk, sit, use hands to finger, handle, feel, reach, balance, see, talk, hear, and drive. Must be reasonably capable of lifting over twenty (20) pounds.
    • Significant portion (>50%) of daily assignments may require travel to client/resident/patient locations or other work sites, via car or public transportation.
    • Work schedule may occasionally require evening, night, holiday, or weekend assignments.
    • Ability to effectively work from a remote/home office.
