This role exists to expand community awareness, support census growth, and ensure a smooth, timely referral-to-admission process.Key responsibilities include developing relationships with hospitals, physician offices, case managers, and community partners; educating referral sources on services and eligibility; tracking and following up on leads; coordinating with intake/clinical teams to remove barriers; documenting outreach activity and results; and representing the organization at meetings and events.Required qualifications/skills:- Experience in healthcare sales, home health, hospice, or related referral development- Strong communication, organization, and follow-through- Working knowledge of referral workflows and compliance expectations- Valid driver’s license and ability to travel locallyApply today to be considered for this full-time, onsite opportunity in Tempe.A Typical DayYour day is spent out in the East Valley connecting with referral partners, balancing scheduled touchpoints with responsive follow-ups.
