POSITION TITLE: Home Health Coordinator
DEPARTMENT: Quality Management
NATURE OF POSITION: The registered nurse (RN) participates as a member of the health care team to provide care coordination for the patient and acts as the patient advocate and a mechanism through which teams of health care professionals work together to ensure that their patients' health needs are being met and that the right care is being delivered in the right place, at the right time, and by the right person.
The Home Health coordinator assists the organization in coordinating the care related to the needs of patients and assists the organization by following home healthcare rules and regulations for the appropriate use of home health services.
CLASSIFICATION: Non-Exempt
SUPERVISION RECEIVED: Director of Quality Management
MINIMUM QUALIFICATIONS:
- Must possess a valid unrestricted Oklahoma license as a Registered Nurse.
- Basic Life Support (BLS) Certification
- Minimum 5 years experience in Home Healthcare nursing including review of
certifications/recertifications.
- Fundamental knowledge of home healthcare rules and regulations
RESPONSIBILITIES: The Home Health coordinator ensures the continuity of care by working alongside the healthcare providers. The Home Health coordinator will:
- Review of home health certifications/re-certifications for required documentation
- Act as a liaison between the organization and home health companies
- Act as a patient advocate
- Attend clinic mandated and departmental in-services and trainings.
- All other duties as assigned.
