Position Overview
The Travel Registered Nurse (RN) provides skilled home health and hospice nursing care to patients across assigned counties. This role includes comprehensive OASIS assessments, wound management, PICC line care, and all professional RN duties required to deliver high‑quality, patient‑centered care in the home setting. The RN works independently in the field while collaborating with the interdisciplinary care team to ensure optimal patient outcomes.
Top Responsibilities & Daily Duties
Clinical & Nursing Care
Perform comprehensive OASIS assessments in accordance with CMS and home health guidelines
Provide skilled nursing care including, but not limited to:
Wound assessment and wound care management
PICC line care, maintenance, and monitoring
Medication administration and evaluation
Monitoring patient condition and clinical progress
Identify patient needs and implement nursing interventions consistent with the plan of care
Recognize changes in patient condition and communicate findings promptly to the care team
Home Health & Hospice Responsibilities
- Deliver compassionate care that aligns with home health and hospice standards
- Support patients and families with education related to diagnosis, treatment, and care plans
- Assist with symptom management and comfort‑focused care when applicable
- Ensure patient safety and adherence to clinical best practices
Documentation & Compliance
- Accurately document all patient visits, assessments, and interventions in accordance with agency and regulatory standards
- Complete clinical documentation in a timely and thorough manner
- Maintain compliance with state, federal, and organizational policies
Care Coordination & Travel
- Coordinate care with physicians, case managers, and interdisciplinary team members
- Travel daily to patient homes throughout assigned counties
- Visit multiple patients per shift as scheduled
Required Qualifications & Experience
- Active Registered Nurse (RN) license
- CPR certification required
- Ability to perform all duties of a professional RN in a home‑based setting
- Willingness to travel within assigned coverage areas

Schedule
- Days: Monday - Friday
- Hours: 8:00 AM - 4:30 PM
- On‑Call / Weekends: None
Mileage Reimbursement
- Yes - mileage reimbursed between visits, from first patient to last patient daily
