Schedule/Location:
Full-time, Monday-Friday 8:30 AM to 5:00 PM, on-site at our Everett office.

What Do We Require of You?
- At least 2 years of intake experience in a medical office.
- Experience as a home health intake coordinator highly preferred, but not required
- Experience with Home Care Home Base highly preferred, but not required
Key Responsibilities on a Home Health Intake Coordinator:
- Perform patient intake, verify insurance, demographics, orders, and document necessary information
- Make persuasive and empathetic outreach calls to patients to explain the value of home health services as ordered by their physician
- Address concerns, clarify expectations, and highlight how care will help them safely recover and regain independence
- Interface with contract, clinical, and office staff to foster collaboration and positive working relationships
- Serve as the primary responder to all inbound calls and inquiries
- Provide excellent customer service to patients, referral sources, and community contacts
- Maintain proper daily workflow and ensure compliance with all regulatory requirements
- Communicate effectively and professionally regarding coordination of care expectations
- Educateand enforce deadlines, schedule updates, and workflow requirements
- Coordinate initial patient assessments and follow-ups with clinical staff
- Ensure accurate data entry and record-keeping for all patient information
- Assist in the development and maintenance of intake procedures and protocols
