The Home Health Liaison represents Arma Home Health Care in the healthcare community and is responsible for developing and maintaining professional relationships with referral sources, including physicians, hospitals, skilled nursing facilities, rehabilitation facilities, assisted living communities, case managers, discharge planners, and other healthcare professionals.
The Liaison supports appropriate referrals, timely communication, and continuity of care while maintaining compliance with California and federal requirements, HIPAA, payer requirements, and Arma Home Health Care policies.
Essential Responsibilities
Develop and maintain professional relationships with current and prospective referral sources.
Educate referral sources regarding Arma Home Health Care services, service areas, admission criteria, and referral procedures.
Identify and communicate appropriate referral opportunities to the intake and clinical teams.
Follow up on referrals and assist with obtaining required referral information and documentation.
Maintain accurate documentation of referral activities, contacts, and follow-up.
Coordinate communication between referral sources, patients/families, intake personnel, and clinical staff.
Represent Arma Home Health Care professionally in the community and at healthcare meetings and events.
Promote patient-centered care and support continuity of services.
Protect patient and referral-source confidentiality in accordance with HIPAA and applicable California privacy requirements.
Follow applicable federal and California laws and regulations governing home health services, healthcare marketing, referrals, and patient rights.
Conduct all activities ethically and in accordance with applicable federal and California restrictions concerning improper referral incentives, remuneration, and healthcare business practices.
Report complaints, concerns, compliance issues, or potential patient-safety issues to the appropriate supervisor.
Perform other duties as assigned by the Administrator or Director of Patient Care Services.
Qualifications
High school diploma or equivalent required; healthcare-related education or experience preferred.
Previous experience in home health, hospice, healthcare marketing, referral development, discharge planning, or a related healthcare setting required.
Knowledge of home health services, referral processes, and community healthcare resources required.
Excellent communication, interpersonal, organizational, and documentation skills.
Ability to work independently and maintain professional relationships.
Ability to travel within the Agency's service area as required.
Must meet all applicable Agency employment and background-screening requirements.
