Home Health Liaison (Sales) - Nassau County, NY

Offering a base salary $60,000 - $90,000 plus a monthly commission plan.  

At AccordCare Home Health, we’re looking for a driven Home Healthcare Liaison sales professional to connect with the community, build lasting relationships, and help families access the quality care they deserve.

• Serve as the friendly, trusted face guiding families into Home Care Services.

• Build and maintain strong referral relationships, driving brand awareness and new client acquisition.

• Nurture long-term partnerships with existing sources.

• Ensure a seamless admissions process by collaborating across departments.

• Stay knowledgeable about AccordCare’s services, requirements, and standards.

• Drive synergy between referral sources, customers, and the internal care team.

Requirements:

• Minimum 2+ years of sales experience in healthcare, private home care, or a related field.

• Proven customer service and sales skills with a relationship-first approach.

• Analytical, adaptable, detail-oriented, and goal-driven.

• Strong communicator with families, partners, and team members.

• Valid driver’s license and ability to travel across Nassau County and surrounding areas.
