Job Summary We are seeking a Home Health Licensed Practical Nurse (LPN) to join our team. In this role, you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You are a compassionate and detail-oriented professional who takes pride in delivering personalized care, observing patient conditions, and reporting changes accurately. To succeed, you should have a solid foundation in nursing practices and a commitment to patient-centered care.
Responsibilities
Provide direct nursing care to patients in accordance with the established plan of car
Assist in evaluating patient needs and communicate findings to the supervising RN
Administer medications and treatments as prescribed, and monitor for side effects or adverse reactions
Document all patient care activities and changes in condition promptly and accurately
Maintain communication with the interdisciplinary team and follow up on care instructions
Educate patients and caregivers on basic health and personal care strategies
Adhere to infection control and safety protocols in the home setting
Qualifications
Currently licensed Practical Nurse (LPN) in the state of Florida
Previous experience in home health or similar setting is preferred
Strong clinical and communication skills
Valid driver’s license and reliable transportation
