Position Summary


We are seeking a Licensed Practical Nurse (LPN) who can balance both administrative and clinical responsibilities. This role involves answering phones in the office, communicating with patients and providers, and providing direct nursing care in patient homes. The ideal candidate is reliable, compassionate, and comfortable transitioning between patient-facing care and administrative support.


Responsibilities


  • Answer incoming calls, assist patients, and communicate with physician offices and clinical staff.

  • Provide skilled nursing care, including wound care, medication administration, IV therapy, and monitoring of vital signs.

  • Assist patients with activities of daily living (ADLs) and personal hygiene to promote safety and comfort.

  • Conduct patient assessments and document findings in compliance with Medicare, Medicaid, and AHCA requirements.

  • Support seamless care coordination with RNs, physicians, therapists, and other providers.

  • Educate patients and caregivers on disease management, medication adherence, and wellness practices.

  • Maintain timely and accurate documentation in electronic health records (EHR).

  • Deliver compassionate care to patients with dementia and cognitive impairments.

Qualifications


  • Active LPN license in the state of Florida.

  • Strong skills in patient education, communication, and organization.

  • Ability to work independently while meeting Medicare, Medicaid, and AHCA standards.

  • Basic knowledge of EHR systems is a plus.
