Position Overview:

The Home Health Licensed Practical Nurse provides skilled nursing care to patients in their homes under physician orders and the supervision of a Registered Nurse. This role focuses on delivering high‑quality, patient‑centered care, supporting disease management, and promoting safety and independence in the home setting.

Schedule & Work Details:

  • Weekly Hour Guarantee: 32 hours
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • On Call and Weekend Requirement: One weekend per month
  • Coverage Area: Shelby County
  • Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient
  • Pay Rate: $28-$30 per hour

Key Responsibilities:

  • Provide skilled nursing care and treatments in the patient's home as prescribed by the physician
  • Educate patients and family members on disease processes, medications, and care techniques to promote improved outcomes
  • Monitor patient condition, identify changes in health status, and promptly report findings to the supervising nurse and physician
  • Document all patient care accurately and timely in accordance with home health policies and regulatory requirements
  • Manage, organize, and maintain patient care supplies and medications to ensure safe and effective care delivery

Required Qualifications:

  • Active Licensed Practical Nurse (LPN) license
