What You’ll Do
- Provide direct, skilled nursing care in patients’ homes under the supervision of an RN, following physician-ordered care plans
- Perform clinical treatments including wound care, medication administration, injections, IV therapy, and vital signs monitoring
- Observe and assess patient conditions, promptly reporting any changes to the RN or physician
- Collaborate with RN Case Managers and interdisciplinary teams to support and update individualized care plans
- Educate patients and families on disease management, medications, safety, and self-care techniques
- Supervise and support Home Health Aides, ensuring quality and consistency of care
- Maintain accurate, timely clinical documentation in compliance with agency, state, and payer requirements
- Participate in case conferences and quality improvement initiatives
- Uphold strict infection control and safety standards during all patient interactions
- Efficiently manage daily travel and visit schedules within your assigned territory
Requirements
What You Bring
- At least 1 year of clinical nursing experience and 1 year of home health experience
- Strong ability to follow physician-directed treatment plans and maintain thorough, accurate medical records
- Excellent communication and interpersonal skills with a patient-first mindset
- Ability to work independently while collaborating effectively with a care team
Required Credentials
- Graduate of an accredited Practical or Vocational Nursing program
- Active LPN/LVN license in the state of practice
- Current CPR certification (must be maintained throughout employment)
- Valid driver’s license and reliable transportation to travel between patient homes
Benefits
Salary: $70,000
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
