Connected Health Care is partnering with a trusted healthcare provider in Kansas City, Missouri to hire a compassionate and dependable Full Time Licensed Practical Nurse (LPN). This position is primarily based in Independence, MO, with occasional travel to Overland Park, KS as needed. Candidates must hold a compact nursing license or active licensure in both Missouri and Kansas.

Licensed Practical Nurse (LPN) Job Responsibilities

• Provide direct patient care under the supervision of Registered Nurses and healthcare providers
• Administer medications and treatments according to physician orders and facility protocols
• Monitor patient conditions and report changes to the clinical team
• Accurately document patient information, treatments, and care plans using Matrix EMR
• Assist with admissions, discharges, and ongoing patient assessments
• Collaborate with interdisciplinary healthcare teams to ensure quality patient outcomes
• Educate patients and family members regarding treatment plans and care instructions
• Maintain compliance with all state, federal, and facility regulations
• Deliver compassionate, patient-centered care in a fast-paced healthcare setting

Education & Certification Requirements

• Graduate of an accredited Licensed Practical Nursing (LPN) program
• Active Missouri LPN license required
• Compact nursing license or active Missouri and Kansas LPN license required 
• Minimum of 1 year of LPN experience highly preferred
• Current CPR/BLS certification preferred
• Experience with Matrix EMR strongly preferred
• Strong communication, organizational, and clinical assessment skills
• Ability to work independently and collaboratively within a healthcare team
