Responsibilities:
Deliver one-on-one skilled nursing care in home settings (wound care, catheter care, medication administration, symptom management)
Perform assessments, evaluations, and skilled observation
Educate patients and caregivers
Collaborate with healthcare team for cohesive care
Requirements:
Current WA LPN license
BLS certification (AHA)
Valid driver's license and insured vehicle
Home Health or Hospice experience preferred (not required)
