Home Health / Hospice LPN

Weekly Guarantee: 32 hours

 Productivity Expectation: 25 points

Coverage Areas: St. Louis City & County, Jefferson County, St. Charles City & County

Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient

Pay Rate: $30-$32/hr


About the Role

We’re seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Clients Home Health/Hospice team. You’ll provide direct clinical care, patient education, and support to individuals managing chronic conditions, recovering from illness or surgery, or receiving hospice services.

 

Key Responsibilities

  • Deliver high‑quality patient care in accordance with the plan of care
  • Perform wound care, dressing changes, and ongoing assessment
  • Provide medication administration and education
  • Manage IV care and maintenance
  • Educate patients and caregivers on disease processes, symptom management, and home‑based care strategies
  • Document all care accurately and timely
  • Collaborate with the interdisciplinary team to support patient goals and outcomes

 Schedule & On‑Call

  • Shift Schedule: To be determined based on operational needs
  • On‑Call/Weekend Requirements: To be discussed during interview

 Required Qualifications

  • Active LPN license in the state of Missouri
  • Previous Home Health or Hospice experience
  • Strong clinical assessment and communication skills
  • Reliable transportation and ability to travel throughout the assigned coverage areas

 Preferred Qualifications

  • IV Certification (highly desirable and will set candidates apart)
  • Experience with electronic medical records
  • Strong patient‑education background
