Home Health Licensed Practical Nurse

About the Role
Anuco Home Health Care LLC is seeking compassionate, dependable, and skilled Licensed Practical Nurses to provide intermittent nursing services in patients’ homes under the direction and supervision of a Registered Nurse.
The ideal candidate has home health experience, strong clinical and communication skills, proficiency with Axxess Home Health EMR, and a commitment to completing accurate documentation within agency-required timeframes.
Responsibilities
- Provide nursing services, treatments, and preventive or rehabilitative procedures according to physician orders and the established plan of care.
- Implement the nursing plan of care under the direction and supervision of a Registered Nurse.
- Administer medications and treatments as ordered and within the LPN scope of practice.
- Monitor vital signs, symptoms, medication effectiveness, treatment response, and the patient’s physical or emotional condition.
- Immediately report significant findings, changes in condition, medication concerns, falls, hospitalizations, incidents, complaints, or unmet needs to the supervising RN and Clinical Manager.
- Prepare equipment and supplies and maintain appropriate infection-control and aseptic techniques.
- Assist patients and caregivers in learning appropriate self-care techniques.
- Educate patients and caregivers regarding medications, treatments, safety, and the plan of care.
- Maintain accurate and complete records of observations, treatments, education, and care provided.
- Accept and document orders as permitted, with required RN review or cosignature.
- Communicate scheduling changes and incomplete assignments to the supervising RN or Clinical Manager.
- Coordinate with the RN and interdisciplinary team regarding patient needs and referrals.
- Participate in case conferences, staff meetings, medical-record audits, QAPI activities, and required training.
- Provide on-call, weekend, or holiday coverage when assigned.
- Protect patient confidentiality and comply with HIPAA and agency policies.
Axxess and Documentation Expectations
- Complete every visit note accurately in Axxess within agency-required timeframes.
- Document the skilled service provided, patient assessment findings, treatment response, education, care coordination, and progress toward goals.
- Ensure documentation is consistent with physician orders, the plan of care, authorized frequency, and assigned duties.
- Submit scheduling changes, missed-visit notifications, incident reports, and coordination notes promptly.
- Respond to QA corrections and return corrected documentation within the requested timeframe.
- Notify the office immediately if a clinical, scheduling, or technical issue may delay documentation.
- Understand that incomplete or late documentation may delay care coordination, authorization, billing, and future assignments.
Requirements
- Current, unrestricted Illinois Licensed Practical Nurse license.
- Graduate of an approved practical nursing program.
- Minimum of one year of nursing experience preferred.
- Home health experience strongly preferred.
- Axxess Home Health EMR experience preferred.
- Current CPR or BLS certification.
- Valid driver’s license, reliable transportation, and current automobile insurance.
- Professional liability insurance as required.
- Strong observation, communication, organization, and time-management skills.
- Ability to work independently while following RN supervision and the established plan of care.
- Ability to travel to patient homes and perform essential duties with or without reasonable accommodation.
Clinical Territories
Applicants may select one or more territories they can cover consistently.
North Territory: Des Plaines, Grayslake, Mount Prospect, Palatine, Glendale Heights, Schiller Park, Carol Stream, Norridge, Niles, and north/northwest Chicago.
Territory assignments depend on patient location, clinician availability, payer requirements, clinical competency, and agency needs.

- Respond promptly to agency communications and QA corrections.
- Demonstrate dependable attendance, sound judgment, and professional conduct.
- Provide compassionate, respectful, and patient-centered care.
