Home Health LPN Visit Nurse

Cooperative Home Care is seeking a compassionate and dependable Licensed Practical Nurse to provide skilled nursing care to patients in their homes on an intermittent basis. You will provide care in accordance with physician orders and the patient's plan of care under the direction and supervision of a Registered Nurse (RN).

Qualifications

• Graduate of a state-approved Practical Nursing program..
• Minimum of one year of nursing experience preferred.
• Current CPR certification.
• Ability to work independently and exercise sound judgment.
• Strong communication, organization, and time-management skills.
• Commitment to providing compassionate, patient-centered care.

Responsibilities 

• Provide skilled nursing services and treatments according to physician orders and the patient's plan of care, while monitoring and reporting changes in condition, symptoms, or reactions to the supervising RN and physician.
• Educate patients and caregivers on care needs, treatments, and appropriate self-care techniques.
• Maintain accurate and timely patient documentation and participate in care conferences, care coordination, staff meetings, in-services, audits, and quality assurance activities as assigned.
• Maintain patient confidentiality, clean bag technique, and a safe, clean work environment while following all Agency policies and procedures.
• Demonstrate competency in nursing skills, report incidents or concerns promptly, maintain reliable attendance, and participate in on-call duties, including nights, weekends, and holidays, as assigned.
