Key Responsibilities
  • Lead and manage home health operations
  • Supervise nursing, PT, and OT services in home settings
  • Hire, train, coach, and develop home care staff
  • Ensure care plan execution and documentation accuracy
  • Monitor and improve quality of patient care
  • Oversee care delivery in:
    • Patients' homes
    • Nursing homes
    • Board & care facilities
  • Drive workflow, compliance, and care standards
  • Participate in interdisciplinary team (IDT) coordination
Mandatory Requirements
  • RN License (California)
  • Minimum 2+ years nursing experience in:
    • Home health
    • OR elderly/frail patient care
  • At least 1 year leadership experience (preferred)
  • Bachelor's Degree (or equivalent experience)
