Community Liaison Home Health Services

A rapidly growing home health provider is seeking an experienced Community Liaison to support referral growth and strengthen healthcare partnerships throughout Port St. Lucie and the surrounding Treasure Coast area. This position plays a key role in expanding market presence, increasing patient admissions, and maintaining strong relationships within the local healthcare community.

The organization provides skilled home health and private duty services designed to help patients recover safely and comfortably in their homes. By partnering closely with physicians, hospitals, rehab centers, assisted living communities, and case managers, the team delivers coordinated care focused on improving patient outcomes and reducing unnecessary hospitalizations.

The company is continuing to expand across the Treasure Coast and offers strong operational support along with long-term growth opportunities.

Primary Responsibilities

  • Develop and maintain referral relationships with physicians, hospitals, SNFs, ALFs, rehabilitation centers, and healthcare professionals

  • Drive new patient admissions through strategic territory management and consistent outreach efforts

  • Execute growth initiatives designed to increase census and strengthen market visibility

  • Conduct in-person meetings, networking visits, lunches, and community outreach activities throughout the assigned territory

  • Collaborate with intake and clinical teams to support smooth patient transitions into home health services

  • Educate referral sources on available programs, services, and admission criteria

  • Attend local healthcare networking events and industry functions

  • Track referral activity, account development, and performance metrics

  • Work closely with leadership to identify market opportunities and referral trends

  • Ensure all activities remain compliant with healthcare regulations and company standards

Qualifications

  • 1 3+ years of home health marketing, healthcare sales, or business development experience required

  • Proven ability to generate referrals and increase admissions

  • Existing relationships within the Port St. Lucie or Treasure Coast healthcare market preferred

  • Knowledge of Medicare and skilled home health services strongly preferred

  • Excellent communication and interpersonal skills

  • Highly motivated with the ability to work independently in the field

  • Strong organizational and follow-up abilities

  • Valid driver's license and reliable transportation required

Compensation & Benefits

  • Base salary: $70,000 $90,000 depending on experience
