Position Details:
Locations: Patients' homes in Jersey City and/or Lebanon, NJ
Pay: $42.00 - $54.00 per hour
Key Responsibilities:
Deliver high-quality nursing care to patients in their homes.
Conduct comprehensive health assessments and develop personalized care plans.
Administer medications and treatments as prescribed.
Monitor patients' progress and adjust care plans as needed.
Educate patients and families on condition management and wellness strategies.
Collaborate with other healthcare professionals for coordinated care.
Maintain accurate and timely documentation of care activities.
Experience & Skills Required:
At least 2 years of nursing experience in a home health setting.
Expertise in trach and vent care and tube feeding management.
Experience working with children and families.
